Where is the B4A on the Ballot and what is its Question Number?

Here is the list of cities, towns and districts which will vote on the question. (Word format) The question number will range from #4 to #7 depending on the town and district; the number is written after the town. In a few places, the question is on the ballot twice, because it is on in both the state House and state Senate district. If you live in those areas, you'll have the privilege of voting for the Budget for All twice!
